Clearance Procedures for JHS Sports

New York State law requires all students participating in athletics to receive a sports physical completed by their physician.  All physicians who administer a physical are required to complete the Longwood Central School District forms.  

The following must be completed before your child is able to participate in athletics:

*Health Certificate/Physical Form - both sides are to be completed, signed and stamped by your child's physician.  All innocent heart murmurs, allergies, asthma, etc. can be addressed on the front of the physical.  Medication orders can be included on the form (i.e. inhalers, epi-pens).  If your child's physician deems your child capable of administering their own medication, he/she can check off the appropriate box, sign an attestation form and PARENT MUST SIGN THE FORM below the medication order.  The physical is good for one year from the date of the exam.

*Health History Form - this is a two page form that parents/guardians must complete, sign and date within 30 days of the start of the sport and must be completed for each sport the student plays.  This form and the Health Certificate/Physical form are necessary and must be handed in together to the Health Office.

*Physicals are available in the health office and are done on a first come, first serve basis once the Health History form is turned in to the Health Office.

*Medication Form

PLEASE REMEMBER NO PHYSICAL OR PAPERWORK WILL BE PROCESSED ON THE SAME DAY IT IS TURNED IN.
